The Nebraska
Chronic Disease Prevention & Control Program (CDPCP) utilizes federal funding to collaborate with federal agencies, state programs, and community partners to improve the prevention and control of heart disease, stroke, diabetes, and cancer along with associated risk factors including: hypertension, high cholesterol, pre-diabetes, overweight/obesity, physical inactivity, and unhealthy eating.
