Data Exchange with Public Health
This page is a resource to Practitioners and Health Care Organizations who wish to exchange healthcare data with Nebraska DHHS, those participating or wishing to participate in the Merit-Based Incentive Payment System (MIPS) Promoting Interoperability Performance Category and for eligible hospitals and critical access hospitals (CAHs) participating in the Medicare Promoting Interoperability Program (PIP) or needing information on Reportable Conditions Reporting.
Healthcare data exchange, reporting and interoperability ultimately improve patient care when shared appropriately between healthcare and public health partners.
Steps for Participation
If you are an eligible health care entity or provider who is currently participating with CyncHealth through the sharing of clinical information pursuant to Neb. Rev. Stat. § 81-6,125, please contact support@cynchealth.org to confirm you are meeting CMS requirements.
Step 1
If your facility has never exchanged data with the state OR your facility wants to initiate exchange of additional types of data: Complete the registration form to select the data sets your organization is interested in submitting. You can preview the form here to insure you have all of the needed information. Registration with Nebraska is only required once for each data set, not annually.
The registration will take approximately 15 minutes to complete. If you are unable to finish you are able to save your work and will be emailed a unique link to continue at a later time.
If you are submitting registration on behalf of multiple practices or organizations that share an EHR, there is a spreadsheet embedded within the registration form with the required information.
Step 2
DHHS will contact you to discuss next steps.
