​​​​​​​​​​​​​​​​​​​​​​​​Nebraska Department of Health and Human Services (DHHS) Division of Developmental Disabilities (DDD) is working to improve both the quality and delivery of services to people receiving home and community-based services. This includes aged people and those who experience disabilities, including intellectual and developmental disabilities. DDD contracted with Liberty Healthcare Corporation in 2020 to help plan and enact changes to improve services. Liberty Healthcare is not replacing personnel or positions, but is working with DDD to put systems in place to improve services.

​​Nebraska's Quality Project and Liberty Partnership 

DDD and Liberty Healthcare Corporation are working together through a contract centered around quality improvement to make services better and more centered around the wishes and needs of each participant. ​


​​Project Phases​

Phase I: Mortality Review

​Mortality reviews are assessments to look at information gathered about a person who has died. A registered nurse will look at information about the participant's health and safety, and make recommendations as needed to prevent future deaths. 

Liberty Healthcare, in partnership with DDD, started completing mortality reviews for all four waivers in July 2021.

Liberty uses an electronic Quality Management Data System (QIDS) to complete mortality reviews, gather data, and perform trend analysis. Liberty co-chairs the Mortality Review Committee (MRC), which is responsible for monitoring mortality trends and making data-driven recommendations that lead to quality improvements for participants of HCBS waiver services and people living at the Beatrice State Developmental Center.

In March 2022, DDD's internal Quality Improvement Committee (QIC) accepted several system-wide quality improvement recommendations made by the MRC.

Phase II: Critical Incident Reporting

When something unusual and concerning happens, it is important to document, so it can be prevented in the future. This is called incident reporting. Incident reports help find problems before they become bigger. States and providers use this information to change supports and systems to help people live healthier and happier lives.

In May 2022, DDD and Liberty Healthcare implemented a pilot for Critical Incident Management on the CDD and DDAD waivers. Through the success of this pilot, a new Critical Incident Management Process was implemented  November 2022. 

July 1, 2022, DDD and Liberty Healthcare implemented a pilot for Critical Incident Management for the AD and TBI waivers. This pilot is ongoing. Information is available on the AD Provider page​.

Phase III: Quality Framework

Liberty Healthcare, in partnership with DDD, is working to improve the state's quality framework. This focuses on improving outcomes for participants through continuous quality improvement.

​​Liberty Feedback Process

An important part of continuous quality improvement (CQI) is eliciting and responding to feedback from a variety of stakeholders. As a part of our commitment to CQI, Liberty initiated a new feedback process. Stakeholders are encouraged to deliver feedback directly to those it relates to; however, when a more formal process is needed, you can complete the Liberty Nebraska Feedback Survey. This information is collected daily and responded to promptly by Liberty Management. For assistance with this process call (402) 500-6525.
